Affinity Group on Aging: New Models of Coordinated Person Centered Care

Join the Affinity Group on Aging for a program on innovative approaches to health care coordination designed to empower the older adult receiving care and make him or her the focal point of the system. Dr. Bruce Leff, Johns Hopkins School of Medicine will speak on Hopkins’s nationally recognized Guided Nurse Care model, which is getting increasing attention as a strategy for overcoming the fragmented system serving persons with chronic illnesses. 

Dr. Leff will also discuss other innovative Hopkins models such as Hospital at Home. Carol Marsiglia, from the Coordinating Center, Inc will speak on the Center’s new Administration on Aging project: Choices for Independence, a cooperative venture with the Howard County Office on Aging. Choices provides comprehensive care coordination and connects individuals 65 and over to the community-based services they need to remain living at home.  

This program is for Maryland Philanthropy Network members only; lunch will be served.
