Reducing Health Inequities in West Baltimore: RICH 2.0 Collaborative
Please join Maryland Philanthropy Network's Health Funders Affinity Group to learn about the RICH 2.0 Collaborative, a learning collaborative and outreach model to reduce inequities in cardiovascular and mental health for West Baltimoreans. Dr. Yolanda Ogbolu, Dean of University of Maryland School of Nursing, will speak on the ambitious project’s significant milestones and the goals for long-term impact.
Through a two-year Pathways for Health Equity grant, the West Baltimore Reducing Inequities in Cardiovascular and Mental Health (RICH) program began as multi-sectoral collaboration that partnered with community members to address cardiovascular and psychosocial disparities. Now, with an additional 5-year Health Equity Resource Communities (HERC) grant, the project is expanding in geography and scope as the West Baltimore Reducing Inequities in Cardiovascular and Mental Health Collaborative-Stronger Together (RICH 2.0).
The Collaborative continues to be a multisector partnership* across multiple community-and faith-based organizations, academia, and hospitals in Baltimore. In addition to the learning collaborative, interventions include a mobile health program, nurse-led clinics located in under-resourced communities, and a robust community outreach model that allows outreach workers to connect directly with clients to address the social barriers to health.
